Incidental Fees:

COD	7% or a minimum of $50.00 (prepayment will avoid this additional cost)
Residential Fee	$75
Appointment Fee	$30 (will apply if carrier must meet customer time constraints)
Lift Gate Fee	Will be per customer request to carrier if manpower is not available to unload; Customer responsible for requesting lift gate and will pay the carrier when delivery is made

Requirements to Unload Units at Destination:

Accessories	1 man **
Ambassador Series	2-3 men
Beacon Series or Baptistery	3-4 men
Premier Series	4 - 6 men and/or lifting equipment ***
Architectural Series	Must have 2 men and lifting equipment ***
* Truck drivers are not required to assist in unloading at destination
** 1 man should be able to lift 100#
*** Lifting equipment may be forklift/boom truck

Freight quotes are valid for 30 days; if the purchase order is received after that 30-day period another quote will be requested from the carrier and cost adjusted as necessary.
Freight quote requests may be emailed or faxed or submitted online via this form to PFI. Phone requests will not be accepted due to the possibility of miscommunication or misinterpretation of information. Normal response time will be within 24 hours.
